Equipment vendors can now download, complete and return Equipment Code Request forms to us via e-mail. It's a quick and easy way to start the ball rolling!


This standardized form should be completed in accordance with GR-485-CORE, "COMMON LANGUAGE® Equipment Codes (CLEI™ Codes) Generic Requirements for Processes and Guidelines." To purchase a copy of GR-485-CORE online, please visit the Information SuperStore.
